Pedalwise Waiver Form
Please read carefully before signing. By signing this document, you agree to the terms outlined below.
1. Acknowledgement of Risks
I understand that participation in cycling lessons and related activities involves inherent risks, including but not limited to:
- Falling from a bicycle, collisions, or loss of control.
- Physical exertion which may cause injury, illness, or aggravate existing conditions.
- Risks associated with outdoor environments, other road or path users, and weather conditions.
I voluntarily accept these risks and understand that cycling activities cannot be entirely risk free.
2. Health & Fitness Declaration
I confirm that I (or the child under my care) am in suitable physical condition to participate in cycling lessons.
I have disclosed any relevant medical conditions, including but not limited to:
- Asthma
- Epilepsy
- Allergies
- Existing injuries or physical limitations
I understand that it is my responsibility to inform Pedalwise staff of any medical or health-related concerns prior to participation.
3. Waiver & Release of Liability
To the fullest extent permitted by law, I hereby release and hold harmless Pedalwise, its coaches, employees and contractors from any and all liability for injury, loss or damage arising from participation in cycling lessons, except in cases of negligence or breach of statutory duty.
4. Parental / Guardian Consent (Participants Under 18)
I confirm that I am the parent or legal guardian of the named child participating in these lessons.
I give consent for them to participate in Pedalwise cycling lessons, accept the risks outlined above and agree to the terms of this waiver on their behalf.
5. Equipment & Safety
- I will ensure that I (or my child) wear a suitable helmet at all times during lessons.
- I understand that Pedalwise may provide equipment, but that safe and responsible use remains my responsibility.
- I understand that instructors may refuse participation if equipment is considered unsafe or unsuitable.
